Medical Withdrawal Notice — Fasola, A.

The official notice withdrawing Amina Fasola from EATNP Cohort 6 in December 2126, eighteen days after her cohort's first actual traversal. The notice is correct on every line it occupies. The lines it does not occupy are also part of the record. This is the document Idris finds in the institutional archive twelve years later.

1 · Statement of Withdrawal

The Office of Trainee Welfare records the withdrawal of Trainee Amina Fasola from EATNP Cohort 6, effective from the date of this notice. The withdrawal is recorded as medical, on the grounds set out at section 2 below. The withdrawal is not voluntary, not disciplinary, not a leave of absence, and does not retain a return-to-curriculum pathway.

2 · Grounds

The training-affiliated medical officer assesses that continued curriculum participation is incompatible with the trainee's medical standing as of the date of assessment. The specifics of the assessment are filed under separate cover (form TW-MED-2126-219, restricted access). For the purposes of this notice, the ground is recorded as: condition incompatible with continued training.

3 · Relation to Cohort Activity

The Office notes for the record that Cohort 6 completed its first actual traversal event on the night of 28 November 2126, under supervision of EATNP Cohort 3 instructor M. Tolfsen. The event is documented under separate cover (form OPS-2126-COH6-T01). The Office records no relation between that event and the present withdrawal.

4 · Standing

The trainee is released from training obligation with full pay through the end of the calendar quarter. The trainee's standing is recorded as medically withdrawn, in good order. No disciplinary finding. No notation against re-entry to other institutional training programs, subject to medical clearance by an EATNP-approved officer.

5 · Records Retention

Training records are retained in the EATNP archive under standard retention. Medical records are retained under the trainee's name, accessible to the trainee and to subsequent EATNP-approved medical officers only. Cohort-records access policy applies to the document under separate cover.

6 · Family Notification

The trainee has been notified in person on the date of this notice by the Office. Family notification is the trainee's option. The Office does not initiate family notification on medical-withdrawal filings.

Cohort 6 admitted twenty-four. Cohort 6 lost two on 28 November 2126 to an event the instructor reported as nominal. Cohort 6 lost two more before the calendar year was out.
